Our cat had 4 kittens 12 days ago, she has been nursing fine up till today. She meows and complains when the kittens start to nurse. She leaves after less than a minute. She looks red and sore between the teats from the little paws. When should I intervene?

Hmm...it sounds more like she has a case of mastitis developing. Kittens that young shouldn't be able to irritate her to the point of redness and soreness. Please run her to the vet to have him check for an infection of the mammary glands and can start her on oral antibiotics. Her milk will go bad if the infection isn't treated.
